A.Hepatocellular carcinoma (Liver Cancer)

Hepatocellular carcinoma (Liver Cancer) is the most common primary liver cancer which occurs predominantly among chronic liver disease/ cirrhotic patients. Hepatitis B or C virus infection may also lead to liver cancer. People who drink alcohol and are obese are at high risk of liver cancer. Every year, 1.6% of cirrhotics develop HCC according to a study. The incidence of liver cancer is rising in the Indian population
Symptoms of pancreatic cancer:
There is no clear reason for the development of pancreatic cancer. Consider meeting the doctor if have you have a family history of pancreatic disease or any of the above symptoms.

What are the treatment options

These are benign tumors of the liver.
Hepatic adenomas are common among females and are related to oral contraceptive intake. It carries the risk of bleeding/rupture and malignancy. Sometimes it may be multiple.
Liver hemangioma is the most common benign lesion of the liver. It may occur in any age group. It is common among females. Mostly it is diagnosed incidentally during imaging of the abdomen.
Symptoms: Abdominal pain, bloating, vomiting, bleeding, tumor rupture, and shock, sometimes incidentally detected during scanning.
Depends upon the size and nature of the lesion. Hepatic adenoma is usually treated by resection of the lesion. Most hemangiomas are managed conservatively. However giant and symptomatic hemangiomas are treated by resection.

Hydatid cyst of liver

Hydatid cyst of the liver is caused by infection of dog tapeworm. Those who handle dogs as pets are at high risk. Uncooked raw vegetables, salads, and fruits contaminated with dog feces may also be a source of infection. It is managed with albendazole and surgery. Various surgical options like deroofing, cysto-pericystectomy, resection, etc are offered.

Cystic neoplasms of the liver

Cystic neoplasms of the liver are the fluid-filled sacs that grow throughout the liver. These complex liver cysts like biliary cystadenoma are detected mostly on imaging. Sometimes they may present as abdominal pain and distension.

Diagnosis: Imaging tests - CT / MRI, radiographs, Ultrasonography,

Fatty liver

Fatty liver is a condition in which fat gets accumulated in large quantities in the liver. This leads to liver inflammation which is called NASH (Non-Alcoholic steatohepatitis) and damages the liver & creates a scar. These scars lead to liver cirrhosis and failure. Alcohol, obesity, and diabetes are the most common causes of fatty liver
Symptoms: Abdominal pain, distension, jaundice, sometimes asymptomatic
Diagnosis: Blood tests / Imaging tests CT / MRI / Liver biopsy

Liver abscess

A liver abscess is a pus-filled cavity that arises secondary to infection in the intestines or from the biliary tree. It is more common among toddy drinkers. Liver abscess is of two types A. Pyogenic liver abscess- (bacterial) B. Amoebic Liver abscess-(Entamoeba histolytica)
Symptoms: Abdominal pain, fever, nausea, loss of appetite, diarrhea.
Treatment: Antibiotics and Anti-amoebic drugs. Image-guided drainage of the abscess.

Right/left hepatectomy

It involves the removal of the right or left lobe of the liver to give adequate margin clearance from the tumor.

Non-anatomic liver resection

It is the removal of the affected portion of the liver by keeping a margin of 1cm from the tumor without maintaining segmental or sectional anatomy. This surgery is employed for small & exophytic lesions

Right / left extended hepatectomy/ trisectionectomy

The extended hepatectomy/ trisectionectomy is done in cases where there is extensive growth of cancer in the liver or the bile ducts at the hilum, to increase the clearance rate. It is one of the most complex surgical treatments. The liver is divided into 8 segments; of which the resection of segments 5,6,7 &8 are right hepatectomy; the resection of segments 2, 3, & 4 is left hepatectomy and the trisectionectomy involves the resection of either 4 to 8 segments or 2,3,4,5, & 8th segment.

Central hepatectomy

Central hepatectomy involves the resection of the central segments of the liver – 4,5 and 8

Left lateral hepatectomy

It involves the removal of segments 2 & 3.
All the liver surgeries are performed wither open and laparoscopic methods based on the extent of liver involvement
